Job Description:

Intermountain Healthcare is seeking an experienced and compassionate Social Worker to join our multidisciplinary team in Utah. In this role, you will be instrumental in aiding diverse patient populations by providing support, crisis management, and connecting them to essential resources within our health system. Our Social Workers play a vital role in enhancing patient wellness and ensuring cohesive care throughout our hospitals and clinics. Assess patient needs and provide initial counseling and crisis intervention. Develop comprehensive care plans in collaboration with healthcare teams. Coordinate with external agencies for long-term support and resources. Maintain patient records and document all patient interactions. Advocate for patient access to community resources. Educate families and patients on healthcare processes and potential resources. Participate in multidisciplinary team meetings. Strong interpersonal and communication skills Ability to manage high stress situations Knowledge of community health resources Crisis intervention techniques Experience in patient care planning Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Licensed Clinical Social Worker (LCSW) or eligibility CPR certified preferred
