/nShift: M-F 8-4:30 rotating weekends/nScrub Color/Attire - Grey
scrub top and black scrub bottoms/nNo floating/nWeekend coverage
needed - Weekend Requirements: 1-2 weekends a month/nOn Call
Requirements None/nEMR – Meditech/nType of equipment – Bioness;
FES; Biodex/nPatient Ratios – 4-5/1/nNumber of Beds - 15/nUtah
Physical Therapy License; BLS/nPositive, upbeat, encouraging;
engaging; compassionate; motivating/nPhysical therapy evaluations
with varying diagnoses/n75-90 minutes physical therapy treatment
plans; 25-30% of our patients are stroke/neuro/nTeam player with
all disciplines-PT/OT/ST/SW/RN/MD/CRS/PPSC/nDeveloping CARE scoring
with all patients for quality/reimbursement purposes/nNumber of
Staff – 3 FT PT; 2 PRN PT; 4 FT OT; 2 PT OT; 3 PRN OT; 1 PRN COTA;
1 FT ST; 1 PRN ST/nDept: IP Rehab Physical Therapy /n Required
Qualifications Physical Therapist, Rehab Experience: 2 years
PT(Compact Accepted) Certifications: BLS-AHA SSN Required DOB
Required Work History: Should not be more than 90 days of gap in
last 1 year/n Preferred Qualifications Neuro Certification/n/n/n
