Practice Provider-Hospitalist Position Title: Hospitalist Advanced
Practice Provider (APP) Location: Bear River Hospital- Logan
Hospital Department: Internal Medicine Position Type: PRN Work
Schedule: - Bear River: 7-19:00 12-hour shifts, including weekends
and holidays - Logan Hospital 7-17:00 10-hour shift, including
weekends and holidays PRN providers are required to submit
availability for at least two shifts per month. Please note that
shifts are not guaranteed in this PRN role. Job Description Summary
We are seeking an experienced inpatient APP to join our Hospitalist
team at Intermountain Bear River Valley Hospital and Logan Regional
Hospital on an as-needed basis (PRN). The Hospitalist Advanced
Practice Provider (APP) plays a critical role in the management of
adult and pediatric hospitalized patients, with a primary focus on
those on general medicine floors. In this role you will work with a
multidisciplinary team of doctors, case managers, nurses,
dietitians, and pharmacists to provide optimal patient care and
outcomes. Key Responsibilities: Advanced Practice Providers (APPs)
are accountable for the provision of patient care at the top of
licensure, within their scope of practice. Licensed, credentialed
and privileged to perform such duties as: - Conduct comprehensive
assessments, including history and physical examinations. -
Develop, implement, and evaluate treatment plans in collaboration
with attending physicians. - Manage acute and chronic medical
conditions, including initiating appropriate diagnostic and
therapeutic interventions. - Work closely with multidisciplinary
teams, including physicians, nurses, respiratory therapists, and
pharmacists, to ensure coordinated patient care. - Facilitate
communication between patients, families, and healthcare teams,
ensuring that patients and their families are informed and involved
in care decisions and care transitions. - Maintain accurate and
timely documentation of patient encounters, treatment plans, and
progress notes in the electronic health record (EHR) system. -
Ensure compliance with hospital policies, regulatory requirements,
and best practice standards. - Participate in ongoing professional
development and continuing education to stay current with
advancements in critical care and internal medicine. - Engage in
quality improvement initiatives within the hospitalist program and
hospital. - Participate in hospital committees, task forces, and

Qualifications - Licensure/Certification: - Current licensure as a
Nurse Practitioner (NP) or Physician Assistant (PA) in Utah - Board
certification as an Acute Care Nurse Practitioner (ACNP), Family
Medicine Nurse Practitioner (FNP) or Physician Assistant (PA-C) -
DEA Certificate II through V - DOJ Drug Enforcement Division - ACLS
and BLS certification for healthcare providers - PALS certification
for healthcare providers - Experience: - Minimum of 2 years of
inpatient APP experience; hospitalist or critical care setting
preferred. - Pediatrics and Adults. - Skills and Abilities: -
Strong clinical decision-making and critical thinking skills. -
Ability to work independently and as part of a team in a
fast-paced, high-acuity environment. - Excellent communication,
interpersonal, and organizational skills. - Commitment to
patient-centered care and continuous improvement. Physical
Requirements: - Ability to work in a hospital environment, which
may involve exposure to infectious diseases and high-stress
situations. - Ongoing need for employee to see and read
information, labels, assess patient needs, operate monitors,
identify equipment and supplies. - Frequent interactions with
patient care providers, patients, and visitors that require
employee to verbally communicate as well as hear and understand
spoken information, alarms, needs, and issues quickly and
accurately, particularly during emergency situations. - Manual
dexterity of hands and fingers to manipulate complex and delicate
equipment with precision and accuracy. This includes frequent
computer use and typing for documenting patient care, accessing
needed information, medication preparation, etc. - May be expected
to stand in a stationary position for an extended period of time.

Purpose The following guidelines have been established to
standardize expectations and build a cohesive and reliable team.
While shifts are not guaranteed, the expectation is set for PRN
caregivers to actively participate and accept available shifts
regularly. Scope Hospitalist APP PRN Staff Working in the Canyons
Region. Definitions PRN Staff: Pro re nata (as needed) PRN
Caregiver Expectations & Compliance Policy To maintain active
status, all PRN caregivers must adhere to the following
expectations: Scheduling & Availability - PRN caregivers must
submit availability and be available to work at least two (2)
shifts per month. - Shifts are not guaranteed. - Exceptions will be
reviewed on a case-by-case basis. - PRN caregivers are expected to
accept shifts regularly unless shifts are unavailable. - Responses
to shift requests must be made within 48 hours of notification via
email or text. - Shift assignments will be given to the first
caregiver to respond. Meetings & Compliance - Attendance of staff
meetings is expected. If unable to attend, the expectation is that
the meeting minutes are reviewed. - Participation in quarterly
check-ins is required. - Must remain current on CME, certification,
license, and DEA requirements. - Maintain and complete on time
required company education, including RQI. - Compliance with
conflict-of-interest policies is required. Physical Requirements:
Ongoing need for employee to see and read information, labels,
assess patient needs, operate monitors, identify equipment and
supplies. - and - Frequent interactions with patient care
providers, patients, and visitors that require employee to verbally
communicate as well as hear and understand spoken information,
alarms, needs, and issues quickly and accurately, particularly
during emergency situations. - and - Manual dexterity of hands and
fingers to manipulate complex and delicate equipment with precision
and accuracy. This includes frequent computer use and typing for
documenting patient care, accessing needed information, medication
preparation, etc. - and - Expected to lift and utilize full range
of movement to transfer patients. Will also bend to retrieve, lift,
and carry supplies and equipment. Typically includes items of
varying weights, up to and including heavy items. - and - Need to
walk and assist with transporting/ambulating patients and obtaining
and distributing supplies and equipment. This includes
pushing/pulling gurneys and portable equipment, including heavy
items. Often required to navigate crowded and busy rooms (full of
equipment, power cords on the floor, etc). - and - May be expected
to stand in a stationary position for an extended period of time.
